Sick notes

To request a sick note, you must have been seen or spoken to by a clinician and self-certified for 7 days prior. If you are requesting a new sick note after this period, then please book a clinician appointment Monday - Friday from 8am. 

You can request a follow on sick note by either:

• filling out a Sick Note Request Form
• by phone or visit the surgery from 10am to 6pm
• by email stonebridge.surgery@nhs.net

Important Information

Please note, sick notes will need to be reviewed regularly by a clinician and if this is the case you will need to book a follow up appointment.

 Sick/fit note requests can take up to 48 hours to process and complete 

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
